Can I contact a doctor after office hours?
Yes, You are able to contact a doctor after office hours by
calling the same office number (703) 962-7277. This hotline
is available 24/7 for Non-Life threatening emergencies.
When calling, please leave your complete name, contact information
and a brief description of the emergency. A medical staff
will contact you within 30 minutes. No
medical advice or prescriptions will be given over the
phone. For life threatening emergencies call 911.
Can I request a copy of
my medical records for my current physician?
Yes, You can request a copy of your medical records by filling out
and signing the
Medical Records Release Form (CFPC to other Physician)
How
can I get my medical records from my past or current physician?
You can request your medical records from your past or current
physician by filling out and signing Medical
Records Release Form (Last Physician to CFPC) and taking
the form to your previous physician who should fax your complete
medical record to us.
Do you accept credit
cards and checks?
Yes, We accept Visa, Master Card, Discover, American Express,
Personal Checks (ID may be required) and Company checks.
